Bring every bottle, including supplements and anything from a specialist. The nurse builds one master list, checks it for duplicates and interactions, sets up pillboxes and a written schedule, teaches the routine to whoever manages it day to day, and flags anything that needs a prescriber's attention.
Setting medications up after a discharge
Discharge is when prescriptions get added, stopped, and changed all at once, and it is the highest-risk moment for an error. Kando reconciles the new list against the old one in the first days home, which pairs naturally with our hospital discharge care.
